1. Agreement Forms

• For Oral Paper Presenters (includes Rapid Fire Medical Student Papers)
• For Formal Discussants (invited discussants of paper presentations)
• For Poster Presenters
• For Workshop Presenters

2. Financial Disclosure Form - ALL POSTER/PAPER/WORKSHOP PRESENTERS

• Only required at this time if your original disclosure (submitted with abstract) has changed or if you have not yet completed since September/October 2023.
